Children’s Hospital & Medical Center is pleased to offer access to certain patient health information via a web-based system called Children’s Connect.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to privacy. We reserve the right to revoke access to Children’s Connect at any time for any reason.

Parent or Guardian Proxy Access – Parents and guardians may request proxy access to their child’s health information by completing the Children’s Connect Consent Form and submitting it to Children’s Hospital & Medical Center. Such access will only be granted to parents or legal guardians of the child, and only to the extent that the individual requesting proxy access can demonstrate the legal right to the child’s medical information. If the proxy’s legal relationship with the child changes, the proxy must inform Children’s Hospital & Medical Center immediately by calling their provider office, the support line at 1-844-296-4615, or emailing the Support line at within the business hours of 8:30 a.m. and 5 p.m. Monday through Friday, or sending written notice to Children’s Hospital & Medical Center. Children’s Hospital & Medical Center reserves the right to revoke proxy access at any time for any reason.

Adult Patient Access – A Children’s patient age 19 or older may request access to their own Children’s Connect health information account. An adult patient must complete a Consent Form and submit it to Children’s Hospital & Medical Center.

Restrictions – Children’s Connect does not reproduce a patient’s entire medical record online, but instead provides access to selected information such as prescriptions, basic laboratory results, summary medical history, appointments and other such information. Even within categories of information that are generally made available through Children’s Connect, posting of information is strictly at the discretion of the individual physician.

To comply with applicable laws and regulations, Children’s provides parents and legal guardians with limited proxy access to minors’ health information through Children’s Connect. The scope of such access may also vary depending on the age and legal status of the minor.

Children’s reserves the right to terminate access to Children’s Connect at any time, for any reason or no reason. In addition, a user’s access to Children’s Connect will be terminated if Children’s determines that the user has misused or abused the system, violated any applicable terms or use or used the system for purposes other than those for which it is intended to be used.

E-mail Notifications – Children’s Connect users will be notified via e-mail when there is new medical information to be viewed in Children’s Connect. This means that any person with access to that e-mail will be able to see this notification. This could include the spouse or employer of the parent or guardian or anyone else that can access their e-mail account. Although no private medical information will be included in the notification, new medical information being available may be information that a parent/guardian would not want others to know. The parent or guardian should take this into account when providing an e-mail address.

Please know that if you send a Children’s provider an e-mail communication, it may be shared with Administration or any staff that assist the physician in providing patient medical care. A patient’s confidential medical information on Children’s Connect will be accessible only to appropriate personnel in accordance with Children’s HIPAA privacy and security policies.

Children’s Connect Messaging – Children’s expects to provide a response to electronic inquiries within two business days. For urgent situations, call your provider’s office directly. For emergency situations, dial 911.

With respect to any electronic communications sent by the patient, parent or guardian, we are only able to respond to such communications based on the information provided. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Please do not use Children’s Connect to send any messages requiring urgent attention. Please call the provider if the situation requires immediate attention, or dial 911 if it is an emergency.

I understand that by attaching a file, it will not take the place of a potential visit with the provider and should only be used for non-urgent medical questions. I further understand that if I believe there is a medical emergency, I should go to an emergency department or call 911 immediately.


Your Role in Protecting Medical Information – Your user name and password provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a Children’s Connect user, your role in maintaining the security of your child’s medical information includes: 1) changing your password on a regular basis, 2) keeping your login ID and password confidential, and 3) ensuring any information printed or stored on your computer is secure.

An App (specialized software program downloaded onto mobile devices) may be available from a Third Party which could interact with MyChart (Children’s Connect). You agree that any App Provider of any Third Party App is solely responsible for its content, warranties, and claims that you may have related to the Third Party App. I UNDERSTAND AND AGREE THAT IN NO EVENT SHALL CHILDREN’S BE LIABLE FOR ANY DAMAGES RELATED TO MY USAGE OF ANY THIRD PARTY APP.

Security and Confidentiality – We afford the same degree of privacy and security protection to medical information stored on Children’s Connect as is given to medical information stored by Children’s in any other medium. Children’s is committed to protecting your child’s medical information. We limit employees’ access and ability to enter or view information based upon their role in your child’s care. Firewalls, passwords, encryption and audit trails are further used to safeguard health information. We have taken steps to make all information received from our online visitors as secure as reasonably possible against unauthorized access and use.

For other than general information viewing, Children’s Connect must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Firefox or Internet Explorer versions 6.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available.

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means the data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Children’s by viewing the digital certificate. This certificate verifies the connection between the Children’s server's public key and the server's identification.

Web Site Links – Children’s Connect may offer links to related medical websites not managed by Children’s. These website link(s) are for a patient’s informational purposes only. Children’s does not endorse and has not verified the accuracy of the information on these websites, and the patient should not rely on any of the information found on the websites for purposes of treatment or diagnosis.
